the results of your treatment plan, please follow
these guidelines:
- We are committed to maintaining a courteous atmosphere and keeping our customers needs first. If you are unhappy throughout any stage, please first speak with the staff member with whom you have a complaint.
- If this fails to result in a satisfactory resolution, please address your concern with the Nurse Manager
at

- If the outcome is still not acceptable to you, please contact the Dr. Zaki Anwar’s office. Your complaint will be investigated and brought to a satisfactory resolution.
